Welcome!
We take a holistic, whole-person approach with all patients, treating infants, seniors, and every stage in between.
Patients may be seen first for either a Comprehensive Eye Exam or a Functional Visual Evaluation:
Our “basic” eye and vision exam is comprehensive. We take into account your work/school demands, interests and endeavors. We support you to fulfill your potential with visual comfort and efficiency. Comprehensive care includes lifestyle recommendations, suggestions, and referrals to allied professionals for complementary support.
Our Functional Visual Evaluation is for patients referred to us with multiple factors under consideration; this necessitates extended testing, consultation, and may entail review of records and reports from other professionals.
Consultations are conducted over two visits: “Consult,” 30 min, and “Functional Visual Evaluation and Eye Exam,” 1 hour. (Patients who are minors need not be present for consult.)
Following the consult and evaluation, the doctor spends 1-1.5 hrs reviewing findings, coordinating with team members, and establishing a plan of action.
In some cases, supplementary evaluations are required:
Visual skills, Strabismus, and/or Visual Perceptual testing. If vision therapy is recommended, the number of therapy sessions required for remediation is estimated after the initial evaluation(s) are completed.
Your extended appointment time with the doctor is reserved expressly for you.
A deposit of $45 is collected at the time your appointment is scheduled. This will be applied to the cost of your visit. The reservation deposit will be forfeited if your appointment is cancelled/ rescheduled with less than 2 business days' notice. Please notify us as soon as possible via email if you are unable to keep your appointment: Jane@DrSlotnick.com.
